Your employer issues your annual P60 summary (not HMRC), so the obvious suggestion is to ask them to re-issue a copy.
However this isn’t always possible. Sometimes employers can’t or won’t issue a second copy, or they can only supply electronic documents. Perhaps you don't work there now and they are dragging their feet, or the company ceased trading.
Whatever the situation, all is not lost. You can re-create the P60 from standard pay information and get a copy as a PDF document or printed payroll forms in a choice of popular styles.
The P60 layout is largely unimportant as long as it includes the necessary information detailing your deductions over the pay year. Probably the most recognisable are the Orange Portrait or Blue Landscape P60 documents. Our document styles page details larger images if you wish to compare.
A copy P60 can be created from either gross annual salary or reverse calculated from total take home pay. You’ll also need name and address of both you and the company, along with your national insurance number, tax code, etc.
